Our nurses provide professional care in your home, including health assessments, medication management and administration where appropriate, wound care, diabetes support and clinical care plans.
We also coordinate with your other health professionals and provide education for you and your support team, so everyone is across your needs and your care stays consistent.
Personal care is deeply individual, and we treat it that way. We provide respectful, dignified support with daily routines including showering, dressing, grooming, toileting, personal hygiene, mobility around the home and meal preparation.
We take the time to understand how you like things done, your preferred routine, how you like to communicate and how much assistance feels right, so you always feel comfortable, confident and in control.
A safe, clean and organised home makes everyday life more comfortable and manageable. We provide practical household assistance while encouraging confidence and independence with household tasks where possible.
Support may include cleaning, laundry, folding and ironing, dishwashing, kitchen cleaning, changing bed linen, grocery shopping, meal preparation and organising your home. We work with you to find the right level of support: completing tasks together, offering reminders and encouragement, or providing more hands-on help when you need it.
Being part of your community can build confidence, connection and independence. We support participants to take part in the activities, events and experiences that matter to them, at their own pace and in a way that suits their goals.
Our team gets to know what you enjoy, what you want to achieve and what feels comfortable for you. From there, we provide tailored support that helps you feel confident, safe and genuinely part of your community.
Being part of the community looks different for everyone. We support participants to attend appointments, social outings, recreational activities, hobbies, local events, community programs, shopping trips, cafes, libraries, gyms, sporting activities and visits with family or friends.
For participants who want to build confidence in public settings, develop social skills, meet new people or become more involved in their local community, we provide patient, encouraging support tailored to their interests, goals and preferred pace.
We support participants to access group programs, centre-based activities, social opportunities and community events that encourage connection, confidence and independence. This may include hobby groups, exercise classes, art programs, community centres, social groups, recreational activities and other activities that align with their interests.
We support participants to feel comfortable in group settings, build social confidence and develop new skills.
Reliable transport support makes everyday life easier and keeps you connected to your community. We can assist with travel to medical and allied health appointments, work, education, shopping centres, social outings, recreational activities, family visits and other important destinations.
Our team can also provide support while you’re out, helping you feel safe and confident. Where appropriate, we can support participants to build confidence using public transport and navigating their local area.
We support participants to build practical skills that can increase independence, confidence and choice in everyday life. Supports are tailored to each participant’s goals, current abilities and preferred learning style, so every session feels relevant and achievable.
This may include support with cooking, meal planning, grocery shopping, budgeting, managing household tasks, organising appointments, establishing routines, personal care, communication skills, travel training, using public transport, building confidence in the community and planning for greater independence. We work alongside you to practise new skills and work towards the goals that matter most to you.
